The fee for requesting personal medical documents for use outside the medical office, and those which are not requested on behalf of the patient by another provider and/or medical facility will be charged a $25.00 document processing charge for official copies.

The fee for missed appointments is $80.00. Please notify the office at least 24 hours in advance if you need to cancel or reschedule your appointment. All appointments canceled less than 24 hours prior to the day of the appointment will incur a $45.00 late cancellation fee. These fees are not covered by your insurance.
